ChatGPT: Error in Moderation

ChatGPT runs every user message through a moderation system before generating a response. When the moderation API call fails — due to a network issue, a server-side problem with the moderation service, or the message being flagged — this error is returned. It can appear for perfectly benign messages if the moderation service itself is experiencing issues, not just when content is flagged. During OpenAI outages, the moderation service sometimes fails independently of the main generation service.
